md: Avoid namespace collision with bitmap API

bitmap API (include/linux/bitmap.h) has 'bitmap' prefix for its methods.

On the other hand MD bitmap API is special case.
Adding 'md' prefix to it to avoid name space collision.

No functional changes intended.
