Clean up Field::CreateFromArtField().

Remove code dealing with the DexCache. This was obsolete
since switching to hash-based DexCache arrays. This makes
the last remaining template parameter unused, so remove it
and move the implementation to the .cc file.

Test: m test-art-host-gtest
Test: testrunner.py --host --optmizing
Test: aost_taimen-userdebug boots.
